Registration & Health Forms
- NJ 4-H Horse Registration Form (PDF) – Required for each project animal.
- NJ 4-H Horse Lease/Borrow Form (PDF) – Required for each registered project animal that is not family/personally owned.
- NJ 4-H Horse Health Information Form (PDF) – Use this form to record required vaccinations and health information for county events and shows. A licensed veterinarian must complete and sign the form.
Measurement & Identification
- NJ 4-H Pony/Horse Measurement Card (PDF) – Official New Jersey 4-H measurement card required annually for all horses, ponies, and VSE's age 6 and younger.
